- Application for admission in any form;
- Original document on education;
- 4 photos 3х4;
- Medical certificate of the form 075-U with the application on fluoroscopy (for disabled persons І and II groups and disabled since childhood, the conclusion of medical and social expertice), approved by order of the acting. Phe Minister of Health of the Republic of Kazakhstan dated November 23, 2010 № 907 (registrated in the Pegister of state registration of regulatory legal acts under № 6697);
- Certificated of unified National Pesting or Comprehensive Pesting (if available);
- Identity document (for identification).
- Documents certifiying the identity of the service recipient are presented personally or by legal representatives.
Service customers – foreigners and stateless persons also sabmif a document defining their status, with a mark of regictration by place of residence:
- Foreigner- residence permif of an alien in the Republic of Каzаkstаn;
- stateless person – certificate of state less person;
- refugee – refugee certificate;
- asglum sceker – asylum sceker certificate;
- оrалmаn – оrалmаn certificate.
